There are incidents and near misses every day, including in the crane industry. Thankfully, they remain relatively rare, but incidents with major consequences do happen. In many cases, the most natural reaction is to resolve the problem and then forget all about it.

MINOR CAUSE
MAJOR CONSEQUENCE

Many incidents are caused by unwanted behaviour, whether conscious or not. Many reactions to incidents focus on tackling the symptoms without ever taking away the cause. But sticking your head in the sand only ever yields a temporary solution. It’s essential that a thorough incident analysis be carried out to help ensure a continuous process of improvement.
What’s more, incident analyses can also be used to establish a database on which measures for improvement can be based. The aim is ultimately to create a safe(r) working environment in which incidents within your own sphere of influence can be prevented.

Investigating the cause of the incident

  • Holding interviews with those involved/witnesses -> 5x WHY
  • Collecting information and data
  • Investigating similar incidents in the past


Learning from previous incidents

  • Making recommendations as to what working methods and procedures should be changed

Preventing similar incidents from occurring in the future

  • Instructing personnel on how to prevent incidents in the future
